Good afternoon Shadow Cliff Parents-

Thank you for taking the time to attend our parent meeting yesterday. As mentioned in the meeting, you will need to fill out this swimmer verification form for ALL of your swimmers. If your child swam for a club team this year, you will be required to provide their USA Swimming number and the club they swim with. Even if your child is no longer swimming for a club team, registration is good through December, so you will still need to provide that information. I can help you locate that information if you need assistance. If your swimmer swam on a summer league team other than Shadow Cliff last summer, you will need to provide that team name as well.
